• Social Security number
• Date and place of birth
• Father’s name
• Mother’s maiden name
• Survivors (name/relationship/city or town of residence)
• Veteran’s discharge papers (DD-214)
• Recent photograph
• Highest level of education
• Occupation (before retirement)
• Place of burial (if applicable)
• Clothing (all items normally worn to leave home)
• Clergy name and phone number
• Insurance policies (if applicable)
